F/T RESIDENCE MANAGER/QIDP/NASSAU COUNTY
The Residence Manager is responsible for the day-to-day operation of the residential program and provides overall leadership, vision and direction in the areas of consumer services, program development, physical plant management, regulatory compliance, strategic planning and program staffing.
BA/BS in Human Services or related field, a minimum of three years' OPWDD experience with at least one year in a supervisory capacity, a valid/clean NYS driver's license and the ability to lift/transfer adults. The ideal candidate is highly organized and able to handle multiple priorities; able to multi-task, work under pressure in a fast-paced environment & meet deadlines; and able to work flexible hours including late evenings, early mornings, overnight and weekends.
F/T ASST RESIDENCE MANAGER/NASSAU COUNTY:
This position requires a high school diploma/GED and a minimum of three years' OPWDD experience or BA/BS in Human Services or related field and a minimum of one year OPWDD experience.
Medicaid Service Coordinator (F/T): BA/BS in Health or Human Services; valid/clean NYS driver's license; minimum of one year experience in an OPWDD or case management/human services related field. Must be willing to travel throughout Nassau and Suffolk to program sites and meetings.
Part-Time At-Home Community Habilitation Specialist in Nassau and Suffolk Counties.
Under the general direction of the Community Services Coordinator, the In-Home Residential Habilitation Support Professional is responsible for providing services to the consumer in accordance with the established Individual Service Plan as well as the In-Home Residential Habilitation Plans. This dedicated candidate will be responsible for providing training and support for the individual in accordance with the person’s In-Home Residential Habilitation Plans; and assisting the individual in the planning, coordinating and attending community inclusion activities within his/her home and community.
High School Diploma or a GED, a college degree in Human Service, Healthcare Administration, Psychology, or related field preferred. A minimum of one-year experience providing direct care support to intellectually challenged individuals preferred. Clean and Valid NYS Driver's License. The ideal candidate is highly organized and able to handle multiple priorities; able to multi-task, work under pressure in a fast-paced environment and meet deadlines; and able to work flexible hours including late evenings, early mornings, overnight and weekends.
Mental Health Clinic
SW/Clinic Supervisor: FT (includes some evenings and saturday's) this position requires a LCSW and a minimum of three years' experience working in a clinical MH setting with children (age 6-18 preferred).
