Residential Program

FULL-TIME AND PART-TIME DIRECT SUPPORT PROFESSIONAL POSITIONS AVAILABLE AT OUR NASSAU AND SUFFOLK COUNTY PROGRAMS.  WEEKEND, OVERNIGHT, MORNING AND EVENING SHIFTS AVAILABLE IMMEDIATELY. 

 

Under the general direction of the residential management team, the Direct Support Professional is responsible for providing direct care to the individuals we serve in such a manner that will promote their independence and individuality.  Minimum requirements include:

  • A verifiable HS Diploma or GED
  • A valid and clean (8 or less points) NYS Driver’s License
  • The ability to attend the Agency's mandatory Orientation Program
  • The right aptitude and a positive attitude
  • A minimum of 6 months experience working with individuals with intellectual challenges a plus, but not required
  • CPR certified and SCIP-R certified a plus, but not required

F/T Residence Manager at our program located in Farmingdale, NY. 

 

Under the general direction of the Assistant Program Director, the Residence Manager is responsible for the day-to-day operation of the residential program.  The dedicated candidate will be responsible for providing overall leadership, vision and direction in the areas of consumer services, program development, physical plant management, regulatory compliance, strategic planning and program staffing.

Requirements:

High School Diploma or a GED.  A college degree in Human Service, Healthcare Administration, Psychology, or related field preferred.  A minimum of one year experience managing an OPWDD certified setting; and a minimum of two years experience providing direct care support to individuals with intellectual challenges.  Clean and Valid NYS Driver's License.  The ideal candidate is highly organized and able to handle multiple priorities; able to multi-task, work under pressure in a fast-paced environment and meet deadlines; and able to work flexible hours including late evenings, early mornings, overnight and weekends.

Under the general direction of the Residence Manager, the Residence Assistant Manager/QIDP is responsible for the day-to-day operation of the residential program.  The Residence Assistant Manager/QIDP will provide overall leadership, vision and direction in the areas of consumer services, program development, physical plant management, regulatory compliance, strategic planning and program staffing.  In addition,  the Residence Assistant Manager/QIDP is responsible for the development, monitoring, implementation, revision and documentation of functional Individualized Program Plans.

Requirements:

A Bachelor's Degree in Human Service, Healthcare Administration, Psychology, or related field.  A minimum of three years  experience  providing direct care support to individuals with intellectual challenges in an OPWDD certified setting.  Supervisory experience preferred.  Clean (less than 8 points) and valid NYS Driver's License.  The ideal candidate is highly organized and able to handle multiple priorities; able to multi-task, work under pressure in a fast-paced environment and meet deadlines; and able to work flexible hours including late evenings, early mornings, overnight and weekends.

 

licensed practical nurse (LPN) for our nassau county programs

 

Under the general direction of the Assistant Program Director and Registered Nurse, the LPN is primarily responsible for the individuals' healthcare.  The LPN will coordinate the medical services provided to the individuals while in the care of the residential program; maintain regulatory compliance; and facilitate training programs.

Requirements:

High School Diploma or a GED and a valid nursing license.  A minimum of one year experience as an LPN, preferably in an OPWDD certified setting. A Valid and clean (8 points or less) New York State Driver’s License and the ability to travel from one location to another within a reasonable timeframe.  The ideal candidate is highly organized and able to handle multiple priorities; detail-oriented; multi-task capable; and able to work under pressure and meet deadlines.

 

Community Services

Part-Time At-Home Community Habilitation Specialist in Nassau and Suffolk Counties. 

Under the general direction of the Community Services Coordinator, the In-Home Residential Habilitation Support Professional is responsible for providing services to the consumer in accordance with the established Individual Service Plan as well as the In-Home Residential Habilitation Plans.  This dedicated candidate will be responsible for providing training and support for the individual in accordance with the person’s In-Home Residential Habilitation Plans; and assisting the individual in the planning, coordinating and attending community inclusion activities within his/her home and community.

Requirements:
High School Diploma or a GED, a college degree in Human Service, Healthcare Administration, Psychology, or related field preferred.  A minimum of one-year experience providing direct care support to intellectually  challenged individuals preferred.  Clean and Valid NYS Driver's License.  The ideal candidate is highly organized and able to handle multiple priorities; able to multi-task, work under pressure in a fast-paced environment and meet deadlines; and able to work flexible hours including late evenings, early mornings, overnight and weekends.

 

 

Mental Health Clinic

Fee for Service Psychotherapist at our Garden City office.

 

Under the general direction of the Director of Clinic Services, the Psychotherapist will provide individual, group and family psychotherapy.   

Requirements:

License Clinical Social Worker.  Experience working with mentally ill patients. Experience working in the substance abuse field a plus.  The ideal candidate is highly organized and able to handle multiple priorities; able to multi-task, work under pressure in a fast-paced environment and meet deadlines; and able to work flexible hours including late evenings and early mornings.
