Full-Time and Part-Time Direct Support Professional Positions Available 


Candidates must have a HS Diploma/GED, and a valid/clean (8 or less points) NYS Driver’s License

A minimum of 6 months experience working with individuals with intellectual challenges a plus 

CPR certified and SCIP-R certified a plus, but not required

Residential Program



This position requires a BA/BS in Human Services or related field, a minimum of one year OPWDD experience, a valid/clean NYS driver's license and the ability to lift/transfer adults.


Community Services

Medicaid Service Coordinator (F/T):  BA/BS in Health or Human Services; valid/clean NYS driver's license; minimum of one year experience in an OPWDD or case management/human services related field.   Must be willing to travel throughout Nassau and Suffolk to program sites and meetings.


Part-Time At-Home Community Habilitation Specialist in Nassau and Suffolk Counties. 

Under the general direction of the Community Services Coordinator, the In-Home Residential Habilitation Support Professional is responsible for providing services to the consumer in accordance with the established Individual Service Plan as well as the In-Home Residential Habilitation Plans.  This dedicated candidate will be responsible for providing training and support for the individual in accordance with the person’s In-Home Residential Habilitation Plans; and assisting the individual in the planning, coordinating and attending community inclusion activities within his/her home and community.

High School Diploma or a GED, a college degree in Human Service, Healthcare Administration, Psychology, or related field preferred.  A minimum of one-year experience providing direct care support to intellectually  challenged individuals preferred.  Clean and Valid NYS Driver's License.  The ideal candidate is highly organized and able to handle multiple priorities; able to multi-task, work under pressure in a fast-paced environment and meet deadlines; and able to work flexible hours including late evenings, early mornings, overnight and weekends.
